About Visiting Nurse Services in Putnam (VNSP)
For well over a century, Visiting Nurse Services in Westchester (VNSW) has been Westchester County’s choice for the provision of home care services. And now, this legacy agency has expanded to create Visiting Nurse Services in Putnam (VNSP) – by community choice. Based on agency integrity, quality of services and fiscal stability, VNSW has been selected by Putnam County officials to provide the home health care services previously provided by the Putnam County Department of Health (PCDOH).
VNSP operates a local office in the Putnam
community, retaining the previous PCDOH staff of nurses, rehab specialists,
social workers, home health aides and clerical staff. The agency’s
comprehensive array of home health services includes nursing, rehabilitation
(physical, occupational and speech therapy services), social work and
home health aide services to start, with several specialty programs to
follow. VNSP‘s clinical
staff utilizes the latest evidence-based protocols, technology and Best
Practice standards for the highest level of care – designed to
help patients return to independence as soon as possible. The parent
agency, VNSW, meets Community Health Accreditation Program’s (CHAP)
Standards of Excellence, with intense oversight provided in
Putnam to ensure the best possible health outcomes and patient satisfaction.
VNSP serves Putnam residents of all ages, with services covered by Medicare, Medicaid and most private insurers. Services are rendered regardless of ability to pay. Unlike other home health care agencies, VNSP develops an individualized plan of care for each patient, with no pre-set number of visits or cut-backs to save money. Each patient receives all the visits needed for the best possible outcome. The agency utilizes a case management approach to ensure that patients receive all services that are needed to reach their maximum level of independence. Our home care nurses serve as case managers and work with the physician, patient and all professional disciplines to establish the plan of care.
VNSP always sees patients within 24 hours – including weekends – despite federal regulations allowing for up to 48 hours. And, the parent agency, VNSW, maintains a nursing supervisor in-house – available during every weekend – to personally assist patients and handle emergencies. VNSW/VNSP has specially-trained nurses in place at hospitals and rehabilitation centers, nursing homes and assisted living facilities – throughout Westchester/Putnam – to ensure our patients’ safe transition home.
